Published in The Journal of Clinical Endocrinology & Metabolism, the research led by UVic PhD graduate Faria Athar reveals a fascinating link between a woman's insulin levels in midlife and the severity and duration of menopausal symptoms.

Athar's study found that higher insulin levels at age 47 predicted an earlier onset of hot flashes and night sweats, and that these levels were also associated with longer-lasting symptoms. This discovery fills crucial gaps in our understanding of menopause and why its symptoms vary so widely among women.

Up to 75% of women experience these vasomotor symptoms, which can begin years before their final menstrual period and persist for a decade, significantly impacting their quality of life. However, the experience is unique for each woman, with variations in onset, severity, and duration. Previous research has shown that factors like race, ethnicity, and socioeconomic status also play a role.

In an interview with Victoria News, Nicole Templeman, Assistant Professor of Biology at UVic and Tier 2 Canada Research Chair in Metabolic Determinants of Reproduction and Aging, highlighted the growing awareness of the connection between metabolic health and reproductive health. Templeman and her team decided to investigate whether insulin levels prior to menopause could provide insights into the incidence and severity of menopause symptoms, given that insulin elevation is often an early sign of metabolic disorders.

The research utilized data from the Study of Women's Health Across the Nation (SWAN), a long-term study that followed women aged 42 to 52 from a pre or perimenopausal baseline for a decade at seven sites across the US. By examining metabolic measurements collected when participants were 47 years old, the researchers discovered that both insulin levels and body mass index (BMI) at this age predicted the timing and duration of vasomotor symptoms.

The strongest association was that higher insulin or BMI was linked to an earlier onset of hot flashes. The analyses also suggested that insulin and BMI may impact hormones and symptoms differently, with insulin being a stronger predictor of hot flashes and BMI more closely associated with changes in estrogen levels.

These findings raise intriguing questions about the potential for lifestyle interventions, such as healthy eating and regular physical activity, to manage or delay vasomotor symptoms by lowering insulin levels. While reversing obesity can be challenging, insulin levels may be more responsive to lifestyle changes, as exercise training has been shown to lower insulin independently of weight loss.

However, Templeman cautions that further research is needed to determine if suppressing insulin levels will indeed lead to meaningful changes in vasomotor symptoms.

"This work establishes a correlative association, but it doesn't necessarily prove that suppressing insulin levels will significantly alter vasomotor symptom incidence," she said.
